On top of the flat aluminum canopy, at the parent pickup in front of Liberty Pines Academy school, located at 10901 Russell Sampson Road, Saint Johns, FL: On or about 5/17/2021, the employer exposed employees to fall hazards of 12-foot to the concrete sidewalk and asphalt drive surfaces, in that fall protection systems were not utilized by employees while installing aluminum canopy and facia materials.

At the west end, north side, of the aluminum canopy at the parent pickup in front of Liberty Pines Academy school, located at 10901 Russell Sampson Road, Saint Johns, FL: On or about 5/17/2021, the employer exposed an employee to over 6-foot fall hazards to the concrete sidewalk and asphalt drive surfaces, in that the 12-foot A-frame step ladder was moved and shifted by the employee working from it while attaching facia materials to the aluminum canopy. b. At the west end of the aluminum canopy at the parent pickup in front of Liberty Pines Academy school, located at 10901 Russell Sampson Road, Saint Johns, FL: On or about 5/17/2021, the employer exposed an employee to over 6-foot fall hazards to the concrete sidewalk and asphalt drive surfaces, in that the 12-foot A-frame step ladder was moved and shifted by the employee working from it while attaching facia materials to the aluminum canopy.
