This condition exposed employees to various hazards associated to but not limited to, ladder and fall hazards due to lack of training and the control or elimination of the hazards.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Gramajo Construction at 4801 Homestead Street, Rapid City, SD 57703: On or about and at times prior to May 18, 2021, the exposing employer did not ensure that employees were protected from falls in that employees were conducting roofing operations on a commercial apartment complex (building #1 - 5015) with no fall protection in place. This condition exposed employees to fall hazards of greater than 23 feet. b) Gramajo Construction at 4801 Homestead Street, Rapid City, SD 57703: On or about and at times prior to May 18, 2021, the exposing employer did not ensure that employees were protected from falls in that employees were conducting roofing operations on a commercial apartment complex (building #2) with no fall protection in place. This condition exposed employees to fall hazards of greater than 9 feet. c) Gramajo Construction at 4801 Homestead Street, Rapid City, SD 57703: On or about and at times prior to May 18, 2021, the exposing employer did not ensure that employees were protected from falls in that employees were conducting roofing operations on a commercial apartment complex (building #3 - 4707) with no fall protection in place. This condition exposed employees to fall hazards of greater than 17 feet. Gramajo Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1280261, citation number 1, item number 1 and was affirmed as a final order on February 9, 2018, with respect to a workplace located at Willow Lakes Golf Course, Offutt AFB, NE 68113.
