29 CFR  1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  (a) Location - 368 Chelmsford Street, Lowell, MA:  On or about 05/20/2021, a pump jack scaffold was erected and used less than ten feet away from energized primary and secondary electrical distribution wiring.

29 CFR  1926.451(g)(4)(vii): Each toprail or equivalent member of a guardrail system was not capable of withstanding, without failure, a force applied in any downward or horizontal direction at any point along its top edge of at least 100 pounds (445 n) or guardrail systems installed on single-point adjustable suspension scaffolds or two-point adjustable suspension scaffolds, and at least 200 pounds (890 n) for guardrail systems installed on all other scaffolds (Reference 29 CFR 1926.452(j)(3)):  (a) Location 368 Chelmsford Street, Lowell, MA:  On or about 05/20/2021, the toprail of a guardrail system on a pump jack scaffold consisted of two pieces of 2x4 dimensional lumber that not were not attached to anything and/or secured to prevent displacement.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed (Reference 29 CFR 1926.451(e)(2)):  (a) Location - 368 Chelmsford Street, Lowell, MA:  On or about 05/20/2021, the ladder used to gain access to/egress from a pump jack scaffold did not extend at least three feet beyond the platform of the pump jack scaffold.
