Worksite: The Employer did not ensure that the Employee who was required to paint from the roof of the residence was protected from fall hazards above 6 feet by the use of guardrails, safety nets or a personal fall arrest system. On or about May 18, 2021, an employee sustained severe injuries after falling from a residential roof while painting.
