29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Lacko Poured Walls located at 115 Beachgate Drive, Granville, OH 43023: On and preceding May 21, 2021, the employer did not ensure that a safe means of egress was located in an excavation that was more than 4 feet deep. An employee was measuring the grade from inside of a basement excavation as it was being dug out. The excavation did not have a ladder, ramp or other means for the employee to egress. The following condition exposed employees to struck-by/caught-in hazards.
