On or about March 24, 2021, an employee was hospitalized after falling from a table. The employer was notified of the injury on or about March 24, 2021. The incident was reported to OSHA on or about May 13, 2021. B. On or about May 7, 2021, an employee incurred amputation of the tip of the left index finger, while performing maintenance activity on a bread slicer. The employer was notified of the injury on or about May 11, 2021. The employer reported the injury to OSHA on or about May 13, 2021.
