29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:  a) On or about May 20, 2021, at the above addressed jobsite, employees performing  roofing operations were not protected from falling, thereby exposing the employees to fall hazards of approximately thirty (13) feet five (5) inches.  Jose Castellanos dba Castellanos Roofing was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1488933  citation number 1,  item number 1 and was affirmed as a final order on December 18, 2020, with respect to a workplace located at 6050 N. Richmond Street, Chicago, IL 60659.
