29 CFR  1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  Location: 20 Bruckner Blvd Bronx NY 10454 - 5th floor  a) On or about May 19, 2021, employees were exposed to fall trip due to the area they were working in having uncovered holes - approximately 4inches.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:  Location: 20 Bruckner Blvd Bronx NY 10454     a) On or about May 19, 2021, employees were using an elevator car rigged to a 2.5 ton hoist to transport employees, materials and equipment between upper and lower floors. They exceeded the hoist's capacity by over 3,000 pounds. Employees were exposed to the hazard of free falling with the elevator car from upper floors to the elevator pit.

29 CFR 1926.552(d): Permanent elevators under the care and custody of the employer and used by employees for work covered by this Act did not comply with the requirements of American National Standards Institute A17.1-1965 with addenda A17.1a-1967, A17.1b-1968, A17.1c-1969, A17.1d-1970, and inspected in accordance with A17.2-1960 with addenda A17.2a-1965, A17.2b-1967:  Location: 20 Bruckner Blvd Bronx NY 10454     a) On or about May 19, 2021, employees were using an elevator to transport employees, materials and equipment between upper and lower floors. The elevator was not inspected. It did not have counterweights installed and it did not have safety devices installed.
