29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Wareroom - The packer conveyor lines had energy sources to include electric, compressed air, and potential energy.  The employer had a written machine-specific Wareroom Packer Cullet Conveyor 1 and Cullet Gate Conveyor lockout/tagout procedure; however, the procedure did not include compressed air as an energy source and so did not include steps for how to de-energize and lockout/tagout compressed air, exposing employees to getting struck-by and caught-in moving machine parts, on or about May 10, 2021.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) Wareroom - Employees did not lockout/tagout when going underneath packer conveyor lines, including the 1D Packer, to troubleshoot a stuck Cullet Gate, exposing employees to getting caught-in and struck-by moving machinery, such as the counterweight on the gearbox for the Cullet Gate's motor, on or about May 10, 2021.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Wareroom - Air valves for packer conveyor lines, including the Cullet Gate on 1D Packer, were underneath the conveyor lines, so employees had to go underneath the machinery to lockout/tagout the air valves, exposing employees to getting struck-by and caught-in moving machine parts when accessing an energy isolating device, on or about May 10, 2021.  Abatement certification and documentation required within 10 days after abatement date. 29 CFR  1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one half the diameter of the shaft:  (a) Wareroom - A motor, 14.5 inches above the work platform, for the inspection conveyor on the 1D Packer, near the operator control station, had an unguarded shaft end that projected 2 inches,  exposing employees to a caught-in rotating parts hazard, on or about May 21, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
