29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:  On or about May 21, 2021, the employer failed to ensure that lockout/tagout procedures were developed, documented and utilized for machinery including but not limited to the low pressure tilt mold #3. Energy sources such as electric, pneumatic, mechanical and gravitational.  NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1517390, citation 1, item 1a, which was affirmed as final order on April 16, 2021 with respect to the same workplace location.

29 CFR  1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures:   On or about May 21, 2021, the employer failed to ensure that employees were trained on effective lockout/tagout procedures based on the programs requirements and any noted deficiencies. Machinery that would fall under this type of program, include but is not limited to the low pressure tilt molds 1-7. Energy sources such as electric, pneumatic, mechanical and gravitational.  NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1517390, citation 1, item 2b, which was affirmed as final order on April 16, 2021 with respect to the same workplace location.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  1) Prepare for shutdown, 2) Machine or equipment shutdown, 3) Machine or equipment isolation, 4) Lockout/tagout device application, 5) Residual/stored energy release, and 6) Verification of isolation.   On or about May 21, 2021, the employer failed to ensure that the application of lockout/tagout procedures were sequentially followed when performing tasks that required the utilization of lock out tag out procedures.  Applicable to machinery including but not limited to the low pressure tilt molds 1-7 with energy sources such as electric, pneumatic, mechanical and gravitational.  NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1517390, citation 1, item 1b, which was affirmed as final order on April 16, 2021 with respect to the same workplace location.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   On or about May 21, 2021, the employer failed to ensure that the lockout/tagout program was periodically inspected to ensure its completeness and effectiveness.  Machinery that would fall under this type of program, include but is not limited to the low pressure tilt molds 1-7.  Energy sources including such as electric, pneumatic, mechanical and gravitational.  NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1517390, citation 1, item 2a, which was affirmed as final order on April 16, 2021 with respect to the same workplace location.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about May 21, 2021, the employer failed to ensure that the low pressure tilt mold was operated by a two hand control, a continuous pressure push button operation or other effective method to ensure the operator is kept free from struck-by or pinch points  hazards created when the mold machine is being closed into operation.
