These chemicals have skin and eye irritating effects.  The employer failed to verify a workplace hazard assessment had been completed.

(a) At a workplace located at 1919 Industrial Dr. Unit 400, Liberty, MO,  on or around 24 May 2021, employees were exposed to electrical shock/fire hazards.  The employer failed to ensure that an electrical receptacle cover was present on an outlet that employees use to plug in a washing machine and to power an electrical strip that is used for cell phone charging and/or operate a coffee maker.
