29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: Jobsite - On or about May 25, 2021, the Gehl telehandler rough terrain forklift was used to support a scaffold platform without being specifically designed by the manufacturer for such use where there was a 16 foot scaffold plank placed on top of the guard rails of the Titan work platform that was being used by employees to install siding from.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level: Jobsite - On or about May 25, 2021, two employees working from the Gehl telehandler work platform were not protected from falling approximately 17 feet to the ground where fall protection was not utilized.
