29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a. 155 Mt. Bethel Road -Villas at Warren, Building 2 The employer did not ensure that employees operating aerial lifts used fall protection. An employee engaged in residential construction activities used a Sky Jack, Model SJ45T, aerial lift for access to install the tin roof and was not using and/or properly utilizing fall protection, where the employee attached the lanyard to the shoulder strap and not the back, center D-ring. Violation observed on or about May 25, 2021

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. 155 Mt. Bethel Road -Villas at Warren, Building 2 The employer did not ensure that employees were provided effective training in the recognition of fall hazards. Employees engaged in residential construction activities used a Sky Jack, Model SJ45T, aerial lift for access to install the tin roof and was not properly utilizing fall protection, where the employee attached the lanyard to a shoulder strap and not a proper D-ring on the back of the harness. Violation observed on or about May 25, 2021
