The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) On or about May 25, 2021, employees were exposed to a fall of approximately 11 feet while performing roofing activities on a residential home located at 102 Firestone Drive, Berea, Ohio 44017. The employees were using an extension ladder to access the roof and the employer did not ensure that it was set three feet above the landing area.
