Office - OSHA 300 and 300A forms for CY 2020 were not used correctly in the following instances: a) The injury sustained on November 28, 2020 was recorded as a Lost Day injury on the OSHA 300 form but was not recorded at all on the OSHA 300A form. b) The work hours recorded on the OSHA 300A form was erroneously recorded as 40 hours when the employees actually worked 50,212 hours.
