29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 30 Bloomingdale Ave. Gloversville, NY - On or about May 19, 2021, employees conducting roofing operations were exposed to a fall hazard from an extension ladder that did not extend three feet above the upper landing. The ladder only extended 1 foot above the upper landing surface resulting in unsafe access and creating a fall hazard of 18 feet .
