29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about May 26, 2021, employees operating pneumatic nail guns without  eye or face protection while installing shingles,  were exposed to struck by hazards that cause puncture wounds and potential blindness or eye injury.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about May 26 2021, employees conducting roofing work on an 6:12 pitch roof on the East side of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eleven (11) feet Three (3) inches at the eave and approximately eighteen (18) feet at the ridge .

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   (a) On, or about May 26, 2021, the employer had not provided fall protection for employee(s) exposed to fall hazards as part of their regular work duties that included fall protection procedures, the use/limitations of fall protection systems, and alternative means of fall protection. This exposed employees to fall hazards greater than 11 feet from the lower level.
