(a)	The employer failed to provide certification certifying the abatement method and completion for frequent and regular inspections of the job site  issued on November 17, 2021 as Citation 1 Item 1.   (b)The employer failed to provide certification certifying the abatement method and completion for operating pneumatic nail guns issued on November 17, 2021 as Citation 1 Item 2.

29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)	The employer failed to provide documentation demonstrating the abatement method and completion for a Werner extension ladder that did not extend issued November 17, 2021 as Citation 1 Item 4 (a) and (b).

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about May 26, 2021, employees were operating pneumatic nail guns without  eye or face protection while installing shingles,  exposing employees to struck by hazard that can cause puncture / impalement injury to the eyes.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about May 26, 2021, employees conducting roofing activities on a single story roof with a 5:12 pitch were not protected from falling to the lower level. This exposed employees on the west side of the roof  to a fall hazard  of  Twelve (12) feet, four (4) inches.  (b) On or about May 26, 2021, employees conducting roofing activities on a second story  roof with a 5:12 pitch were not protected from falling to the lower level. This exposed employees on the North side of the roof  to a fall hazard  of  over twenty (20) feet.  (c) On or about May 26, 2021, employees standing on the  on a second floor  were not protected from falling to the lower level when located next to the unprotected window hole. This exposed employees on the North side of the structure  to a fall hazard  of  over fourteen (14) feet feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about May 26, 2021, a Werner extension ladder used to access the upper landing surface on the West  side of the structure did not extend at least three (3) feet above the upper landing surface, nor was the  ladder secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail,  provided to assist employees in mounting and dismounting the ladder. This exposed the employees to a fall hazard of  twelve feet (12)  four (4) inches.  (b) On or about May 26,2021, a Werner extension ladder used to access the second floor window sill  landing surface on the North side of the structure did not extend at least three (3) feet above the upper landing surface, nor was the  ladder secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail,  provided to assist employees in mounting and dismounting the ladder.  This exposed the employees to a fall hazard of fourteen (14) feet.
