29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about May 25, 2021, the employer did not ensure that employees, installing metal decking and insulation, from the roof of a building were in use of fall protection. This exposed the employees to fall hazard greater than six(6) feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about May 25, 2021, at the jobsite, the employer did not ensure that the side rails of an extension ladder, being used by employees to access the roof, was extended at least three feet above the landing. This exposed the employees to a fall hazard.
