29 CFR 1910.22(a)(3):29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. (a) Suds and Duds, Inc. dba Servall Uniforms at 410 4th Street, Rapid City, SD 57701: On or about and at times prior to May 20, 2021, the employer did not ensure that the walking-working surface was maintained free of protruding objects within the stain treat area. This condition exposed employees to trip hazards. An employee was working within this area, and tripped on a protruding pipe; the employee fell and was fatally injured as a result of the fall.

29 CFR 1910.1200(f)(6):The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: (a)Suds and Duds, Inc. dba Servall Uniforms at 410 4th Street, Rapid City, SD 57701: On or about and at times prior to May 25, 2021, the employer did not ensure that chemicals in the workplace were properly labeled. Employees were utilizing Pyratex for the process of stain removal and treatment and the container being used was not properly labeled. This condition exposed employees to possible injuries associated with improper use of the chemical.
