29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a. On or about May 20, 2021, employees were exposed to stuck-by hazards, when required to operate Toyota Electric Forklift, Model #8FBE20U, Serial No. 13764. The power industrial vehicle did not have a functioning rear lights, brake lights, right turning signal, back-up alarm or blue light warning spotlight, and was and was not removed from service.
