(a) On or about May 27, 2021, an employee working in an approximately ten (10) foot deep trench while installing a water line utility, was not wearing eye protection, exposing the employee to hazards from flying, and/or falling particles from above, or from hazards created inside of the trench while using cordless tools and other equipment.

(a) On, or about May 27, 2021, employees working in an approximately ten (10) foot deep trench installing water and natural gas line utilities, were exposed to struck by hazards from falling rocks, dirt and tools from above. These hazards were not contained or separated by being kept at least 2 feet from the edge of the trench.
