29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extend at least 3 feet (0.9 m) above the upper landing surface: a) South panel of roof, east end, on or about 11-1-21: One employee used a ladder for access to a roof, that was not 3 feet above the landing surface, in that the top of the ladder was 16 inches above the landing surface, exposing the employee to a fall hazard of 14 feet to the concrete pavement below.
