On or about May 27, 2021, on a 5:12 pitch roof, four employees performing re-roofing operations were not protected from a 10-foot 7-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a. On or about May 27, 2021, four employees used a 12-foot Little Giant ladder to access a 5:12 pitch roof that was not 3 feet over the eave, exposing employees to a fall hazard.
