First Floor Cellar of Elevator Shaft Area. a) Employees tripped over pieces of metal that were on the floor surface and fell approximately 8 feet to a steel decking elevator shaft pit work area, while putting on his harness in a controlled access zone. On or about 05/26/ 2021.
