29 CFR 1926.21(b)(2): The employer failed to instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) On or about May 25, 2021, and at times prior thereto, at the construction site, ironworkers were exposed to struck-by hazards from the unexpected re-energization of machinery when performing work in the path of travel of the man hoist.
