29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Signature Label at the location of 280 North Lake Winds Parkway, Oak Harbor, Ohio: On or about May 18, 2021, employees were exposed to in going nip-points created between the orienting disc and the timing belt of the New England Unscrambler. An employee suffered an amputation injury while adjusting a "Native" brand canister cap after it became stuck in the feeder of the New England Unscrambler during production operations.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Signature Label on or about May 18, 2021, at the address of 280 North Lake Winds Parkway, Oak Harbor, Ohio: The employer failed to report an employee amputation following a worksite incident (ring finger of the right hand caught in between the orienting disc and the timing belt of the New England Unscrambler). The employer or employer representative failed to contact the Toledo Area Office or the 24 hour OSHA number to notify the agency of an employees amputation injuries within 24 hours of receiving the injuries. The employer later contacted the OSHA Office on May 25, 2021.
