29 CFR  1926.451(b)(5)(i):The end of a platform 10 feet or less in length, extended over its support more than 12 inches:  a) On or about May 27, 2021 - On the Eastside of the project planks on the outrigger of the scaffold system extended approximately 24-36 inches beyond the support frame exposing the employees to a fall hazard of approximately 24 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(g)(4)(iii):When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform.  a) On or about May 27, 2021 - Where  employees were performing masonry work on the eastside of the site a scaffold had been erected and the employer did not install mid-rails along the working level.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:   a) On or about May 27, 2021 - A toeboard was not installed where employees were working on the fourth level of a scaffold performing masonry work  and where there was  tools or materials  present.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about May 27, 2021 -  An employee standing at the edge of a  roof without fall protection was exposed to a fall hazard of approximately 10 feet while assisting to provide masonry supplies to employees on the scaffold.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
