29 CFR  1910.106(d)(5)(i): Flammable liquids were stored so as to limit use of exits, stairways, or areas normally used for the safe egress of people.  On or about May 28, 2021, and at times prior thereto, a 55-gallon drum containing a flammable chemical was stored approximately 2-feet from an emergency exit, limiting the use of the exit for safe egress, and exposing employees to fire and explosion hazards in the event of an emergency.

29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment.  On or about May 28, 2021, and at times prior thereto, no assessment had been performed to determine what Personal Protective Equipment (PPE) was required to protect employees in the workplace, exposing employees to the hazards of inhaling or coming into contact with hazardous chemicals.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  On or about May 28, 2021, and at times prior thereto, the employer had not provided eye wash stations for employees required to use corrosive chemicals, exposing employees to the hazard of being struck-by hazardous chemicals in their eyes.

29 CFR  1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii).  On or about May 28, 2021, and at times prior thereto, multiple employees were using hazardous chemicals on printing press machinery without the employer's development or implementation of a written Hazard Communication Program, exposing employees to various chemical hazards.

29 CFR  1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible.  On or about May 28, 2021, and at times prior thereto, a UV press operator employee was allowed to voluntarily wear a tight fitting half-face respirator without having been provided basic information regarding respirator safety, exposing the employee to the hazards of increased physiological burden and aggravation of a cardiac or pulmonary disorder.
