On or about 6/1/2021, at 200 Ozark Outdoors Land, Leasburg, MO 65535, the employer failed to assess the worksite to determine the personal protective equipment when the use of the Methyl Ethyl Ketone, lubricants, engine coolant and other chemicals in the maintenance shop.

29 CFR  1910.138(a):The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion:  The employer is failing to protect employee exposed to hazardous chemical. On or about 6/1/2021, at 200 Ozark Outdoors Lane, Leasburg, MO 65535, the employee were permitted to use a variety of automotive chemicals but not limited to lubricants, degreasers and coolant without any form of hand protection. The employer failed to protect their employees by not providing them personal protective equipment.  The employees were exposed to potential skin rashes and  burns hazards.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  The employer is failing to protect employees from laceration and puncture hazards.  On or about 6/1/2021, at 200 Ozark Outdoors Land, Leasburg, MO 65535, the employee were permitted to use place their hand in the danger zone while using a Shop Master Drill Press, Model: SM 300, S/N: 200634XL, 120v, 3100 rpm.  The employer failed to protect their employees by not guarding the chuck on the drill press.

29 CFR  1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  The employer is failing to protect from laceration and puncture hazards.  On or about 6/1/2021, at 200 Ozark Outdoors Land, Leasburg, MO 65535, the employee were permitted to use and place their hand in the danger zone while using a Central Machinery, 6 inch bench grinder, Item #39797, 1/2hp, 3,000rpm.  The employer failed to protect their employees by not adjusting the wheel and work rest to a maximum opening of 1/8 inch.  The employees were exposed to a struck-by hazard.
