The employer did not identify a physician or other licensed health care professional (PLHCP) to perform medical evaluations, did not provide a follow-up medical examination for employees who gave a positive response to any question among questions 1 through 8 in Section 2, Part A of Appendix C, and did not obtain a statement that the physician or other licensed health care professional (PLHCP) had provided the employee with a copy of the PLHCP's written recommendation.
