29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) Steele Brothers Heating, Inc. at 1312 Snowberry Lane in Louisville, CO 80027: On or before 5/25/2021, the employer did not ensure that holes in walking/working surfaces were guarded. Steele Bros. employees were directed to install HVAC systems on 3-story apartment building with an open ventilation shaft adjacent to the elevator shaft, which was not guarded or covered. This condition exposed employees to a fall hazard of approximately 35 feet. An employee fell through the ventilation shaft and suffered an injury.
