29 CFR  1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer did not provide abatement documentation proving that the hazards related to fall protection training certification were abated for the citation that was issued on August 4, 2021.

29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  The employer did not ensure that two employees engaged in cleanup tasks working below other employees wore head protection, exposing employees to struck-by hazards from falling objects.

29 CFR  1926.405(g)(2)(iii):Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.  The employer did not ensure that a flexible cord being used to charge five portable power tool batteries was used only in a continuous length without splice or tap, exposing employees to fire, burn, and shock hazards.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  The employer did not ensure that seven employees working on residential roofing with a 4:12 pitch that was 9 feet 7 inches above a lower level were protected from falling, exposing employees to falling hazards.

29 CFR  1926.503(b)(1):The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  The employer had not completed written fall protection certification records for seven employees who were exposed to various fall hazards at the project site located at1595 Maricopa Drive, Oshkosh, Wisconsin.
