29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. 501 S. 9th St, Reading, PA - An employee suffered an amputation of his left index finger and lacerations to his left thumb on May 20, 2021. The employer did not report the amputation to OSHA until May 24, 2021. No abatement certification or documentation required.
