29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a DFA Dairy Brands Fluid, LLC DBA Meadow Gold at 1325 W. Oxford Ave. Englewood, CO 80110: On or before 5/31/21 the employer did not protect employees from injury in that the access doors on the sides of the N5 filler machine were easily opened with the machine still running. The doors lacked locks, electronic interlocks, or other approved methods of restricting access to the machine while it was running. An operator opened the side door to clear a jammed milk carton and was caught in the moving parts of the machine, losing their left thumb in the process. This condition exposed employees to amputation hazards. Abatement Note: Abatement certification and documentation are required for this item. See enclosed "Certification of Corrective Actions Worksheet."
