29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.   At 6810 S. Hwy A1A in Melbourne Beach, Florida, on or about June 2, 2021, and at times prior, on the 6:12 pitch roofs of the house and garage, three roofing employees installing sheet metal roofing panels were exposed to 8 feet and 18 feet fall hazards in that they were not protected by a fall protection system.  Kuzak Roof Maintenance, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1384742, citation number 1, item number 2 and was affirmed as a final order on September 6, 2019, with respect to a workplace located at 2800 Business Center Boulevard in Melbourne, Florida.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.   At 6810 S. Hwy A1A in Melbourne Beach, Florida, on or about June 2, 2021, and at times prior, on the north eave of the house, the roof access ladder was extended only 24 of the require 36 inches above the roof eave edge, exposing three roofing employees to an 18 feet fall hazard.
