29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about 06/03/2021, four employees at the 21 Tanbark Trail residential housing project were exposed to approximately 10' falls. The employer's ladder did not extend at least three feet above the second-floor landing of the house.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 21 Tanbark Trail, Farmington, CT: On or about June 03, 2021, the employer did not provide guardrail systems, safety net systems, or personal fall arrest systems and did not provide a fall protection plan for its employees. Employees were exposed to fall hazards that were between 14 to 16 feet above the ground. a. One employee standing in an unframed and unfinished second-floor window appeared to be receiving supplies and standing close to a fall hazard without fall protection. b. Another employee was working on unfinished and unframed rafters/roof leaning over the edge to place nails into a gable area without fall protection on the second floor. c. The unfinished and unframed second-floor staircase landing was unguarded, where the ladder was placed, and employees were congregating to go down the ladder.
