29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that would have prevented employee exposure to hazards: Trench located at 72 Thompson Street, Alpharetta, GA 30009: On or about 06/02/2021, the employer exposed employees to engulfment hazards while installing 8-inch ductile pipe, in that the protective system used to prevent potential cave-ins was not installed with the all the required spreaders/struts and was not stacked on top of each other using the corner connectors and pins as indicated in the tabulated data for this equipment by the manufacturer.

29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: Trench located at 72 Thompson Street, Alpharetta, GA 30009: On or about 06/02/2021, the employer exposed employees to engulfment hazards while installing 8-inch ductile pipe, in that the ladder used for egress from the trench was located outside the trench box/shield being used. The employer failed to place the ladder inside the protective system to allow for safe egress.
