Tool Area: On or about 5/19/2021, an employee amputated the tip of his thumb when it was caught between metal stock and the clamp on the horizontal band saw. The employer did not report the amputation until 5/25/2021.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v) (a) Dispensing Area : On or about 6/2/2021, and at times prior, the employer exposed employees to fire hazards in that labels such as pictograms or name of the chemical was not attached to the 5-gallon cutting fluid drum when employees were required to refill secondary containers.
