Jobsite: Fifth floor roof Employees accessing the elevator shaft roof were exposed to a fall hazard of approximately 28 feet.

29 CFR 1926.1053(b)(7):Slip resistant feet were used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces, including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery. Location: Elevator Shaft Roof A ladder set up on the membrane roof's protective film was not prevented from slipping by securing it to the shaft roof, wall or the main roof.
