29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On June 4, 2021, and at times prior thereto, workers engaged in roofing work a steep slope roof (approximately 12:12 pitch) were not using personal fall arrest systems. This included a worker installing roof jack scaffold planks next to a 15 to greater than 20 feet fall from the gable end over the church entrance. Note: Certification of corrective action is required for this item.
