29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) On or about May 26, 2021 - Apollo Street, Huntsville, AL 35808, Building 4707, employee(s) removing ductwork located adjacent to a mezzanine outside of the mezzanine guardrail that served as the ceiling to the floor below were exposed to fall hazards of approximately 10 feet when the guardrail was used as a anchor point while leaning through a guardrail and when walking on joists/cross braces with no fall protection.
