29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wore a protective helmet when working in areas where there is a potential for injury to the head from falling objects: JOBSITE: On or about June 1, 2021, the employer had not ensured that each employee performing tree debris/removal operations were required to wear a protective helmet. As a result, an employee operating the Stihl chain saw without a helmet suffered a fatal injury when the chainsaw kicked back and struck him the head .
