(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling.  a. On or about June 7, 2021, employees loading tumble Mixer 6 and Mixer 7 were exposed to a fall hazard while working from an elevated platform, approximately four feet and eight inches from the concrete floor.   b. On or about June 7, 2021, employees loading tumble Mixer 4 and Mixer 5 were exposed to a fall hazard while working from an elevated platform, approximately four feet and eight inches from the concrete floor.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a. On or about June 7, 2021, the employer did not ensure that junction boxes and outlet boxes throughout the facility were covered, thereby exposing employees to an electrical shock hazard.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a.
