29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. At the shop area: on or about May 28, 2021, for employees exposed to struck by and caught-in hazards while operating the Okuma CNC milling machine, model Genos M560-V without a functioning interlock door device.  b.  At the shop area: on or about May 28, 2021, for employees exposed to caught-in and struck-by hazards created by rotating parts and ingoing nip points while operating the Fadal CNC milling machine, model 906-1 without a functioning interlock door device.   c.  At the shop area: on or about May 28, 2021, for employees exposed to caught-in and struck-by hazards created by rotating parts and ingoing nip points while operating the Fadal CNC milling machine, model 904-1 without a functioning interlock door device.   d. At the shop area: on or about May 28, 2021, for employees exposed to caught-in and struck-by hazards created by rotating parts and ingoing nip points while operating the Mazak CNC lathe machine, model QT-20 without a functioning interlock door device.  e.  At the shop area: on or about May 28, 2021, for employees exposed to caught-in and struck by hazards created by rotating parts and ingoing nip points while operating the Bridgeport manual lathe machine, model Tormax 16-5 without a chuck guard cover.

29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a.  At the shop area: on or about May 28, 2021, employees were exposed to amputation hazards during the cutting of steel tube pieces up to 4 inches in diameter with a Kasto Fuctional A horizontal band saw.  The band saw blade had an unguarded portion of approximately fourteen inches (14") at the point of operation.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a.  In the CNC machine area: on or about May 28, 2021, Flexible cords (extension cords) were being used in lieu of permanent hard wiring to power a lighting fixture on the Yama Seiki Machine, model GA-2600, exposing employees to electrical fire hazards.  b.  In the shop area: on or about May 28, 2021, Flexible cords (extension cords) were being used in lieu of permanent hard wiring to power pedestal fans, exposing employees to fire hazards.
