29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Any Pitch Roofing LLC on or about June 08, 2021, at the site location of 1766 S. Clinton St. Defiance, OH: The employer did not ensure employees were protected from falls while performing roofing installation from elevated surfaces. Employees working from atop a residential structure roof were exposed to fall from height from 9ft. up to 17 ft. while performing roof installation activities without the benefit of fall protection.
