29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  A) Augustine Italian Village Inc., on or about June 03, 2021, at the address of 427 Commerce Avenue, New Castle, PA: The Benier dough roller was not adequately guarded. Employees work within 1 foot from the pinch point between the rollers and the belt that transports the dough between the machines.  A caught-between hazard was present between the upright supports and the moving parts.

A) Augustine Italian Village Inc., on or about June 03, 2021, at the address of 427 Commerce Avenue, New Castle, PA --- Benier dough shaper:  A moving gear hazard was present from a rotating dough shaper. The gears were not physically guarded and. Rotating gear hazards were within 4 feet of the floor.

The wire was damaged, exposing the copper conductor, exposing employees to potential electrical hazards.
