29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)  The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violation that was issued by the Agency.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  (a)  On or about June 8, 2021, employees engaged in re-roofing (tear off) on an existing commercial building with a 5/12 pitched roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about June 8, 2021, the employer did not ensure employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.
