29 CFR  1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.  On or about June 9, 2021, an employee was not protected from an energized power line service, while working/walking on an aluminum pump jack scaffold.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Location: 1603 Main Street, Acton, MA 01742.  On or about June 9, 2021, employees were not provided a safe means of access to the ladder jack scaffold platforms.

Location: 1603 Main Street, Acton, MA 01742.  On or about June 9, 2021, employees were not protected from falling while working/walking on ladder jack scaffolds that were greater than sixteen feet above lower level.

Location - 1603 Main Street, Acton, MA 01742.  On or about June 9, 2021, employees were not protected from falling while working/walking on a roof greater than six feet above lower level.

Location - 1603 Main street, Acton, MA 01742.  On or about June 9, 2021, employees were not protected from falling while using ladders that did not extend at least three feet above the upper landing surface.
