29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. U.S. Postal Service, 433 W. Harrison Street, Chicago, Illinois/Cordiss Collins PDC - On or about June 9, 2021, the employer failed to record the following workplace injury/illness on the OSHA 300 Log for calendar year 2021. a.) 3rd Floor, APBS Area - On or about May 31, 2021, a recordable injury occurred when a mechanical equipment operator drove the Toyota Electric Forklift Truck, Model SFBE15, Serial No. 32805 over the feet of an employee. The injured employee sustained two (2) broken toes on the right foot, and four (4) toe amputations on the left foot.
